User submission not showing up to instructor

This is a really strange bug.  A learner taking the test completed their assignment and submitted it.  She sent in a screen shot show that it was complete and submitted but it shows up on my end as not started.

I checked in the SQL database and she did complete it.

Is this a browser bug or is it something wrong with SLK.  I am running 1.4

Try tracing the calls to the DB with SQL Profiler to see what statement is causing the problem.

 I ran it with everything selected and all I get is the query for the site theme which is belltown.  What am I doing wrong
